Erectile Dysfunction and Treatment

Almost every man may experience erectile dysfunction at least once in his life. If this problem is encountered individually during a certain period, in cases such as work fatigue, stress, and inequality, it should not be a problem. If the problem recurs or worsens, it is inevitable to seek treatment.

It is important to remember that erectile function (the function of male erection and sexual intercourse) is the main building block of happiness that holds the family together. In other words, erectile dysfunction is not just a problem for one person, but for both husband and wife. Given the continuation of the desire to be together, i.e. the institution of the family, a broken arm has a more profound impact on the social fabric than kidney disease or eye disease. Therefore, treatment should be sought at least as much as for other illnesses, if not more.

Cinsel fonksiyon bozuklukları, cinsel cevap döngüsü esas alınarak tanımlanır. Cinsel cevap döngüsünün ise; istek, uyarılma, orgazm ve çözülme olmak üzere dört evresi vardır. İşte “cinsel sorunlar” genel tanımı, bu evrelerin herhangi birindeki aksama ve problemi ifade eder. Erektil fonksiyon bozukluğu, ya da halkımızın yaygın tercihiyle “iktidarsızlık” ise, cinsel cevabın ikinci safhası olan uyarılma evresinde erkeklerde görülen aksamaların adıdır. Erektil disfonksiyonun yaygınlığı hakkında en sağlıklı rakamlar ABD’den alınmaktadır. Buna göre, 40-70 yaş arasındaki erkeklerde, %10 oranında tam ereksiyon bozukluğu, %52 oranında ise değişik derecelerde erektil disfonksiyon mevcuttur. Bunların % 80 kadarı, organik bir nedene bağlıdır. Toplumsal şartlar ve cinsellik kavramına yaklaşım farkı gözönüne alındığında, ülkemizde erektil disfonksiyon ve cinsel sorunların daha yüksek oranda olduğu, ancak bunların altında yatan psikolojik etmen yüzdesinin hatırı sayılır bir rakama ulaştığı söylenebilir.

When we look at the causes of erectile dysfunction; cardiovascular diseases (such as hypertension, high cholesterol), diabetes, spinal cord trauma, urological or general surgical interventions (such as prostate cancer, bowel cancer, bladder tumor surgeries), psychological factors such as depression-stress, some medications used permanently, smoking and alcohol use can be counted as the most important predisposing factors.

Before seeking medical treatment, it is useful to try to improve the problem spontaneously by making daily life changes such as quitting alcohol and smoking, avoiding environments and events that cause stress and anxiety. If the problem cannot be solved in this way, medical help should be sought.

Ereksiyon sorunlarında ilk başvuru adresi “üroloji uzmanı” olmalıdır. Yapılacak hızlı ve basit testlerle sebep hakkında bir kanaat edinilir ve önce ağızdan alınan ilaçlarla, olmazsa penise iğne uygulanarak çözüm aranır. Son çare olan penis protezi (mutluluk çubuğu) ameliyatına gitmeden önce ise, penise şok dalgası uygulaması şeklinde yapılan son derece zararsız ve kolay bir yöntem de denenebilir. Ağrısız, acısız, ilaçsız, iğnesiz, anestezisiz ve yan etkisiz bu yöntemde hastaya 6-12 seans şok dalga tedavisi yapılır ve özellikle hafif-orta şiddetteki vakalarda %70’lere varan başarı sağlanır.
